In the 9th Statement of Work (SOW), QIOs will conduct several activities related to their charge to improve the quality of care available to nursing home patients.

Physicians

Do you have Medicare beneficiaries with diabetes in your practice? Do your Medicare patients with diabetes need assistance in managing their diabetes more effectively? If you answered yes to these questions, the Every Diabetic Counts program is for your patients. As an Every Diabetic Counts practice site provider, your Medicare patients with diabetes will be enrolled in a free diabetes self-management education program in their community. As a practice site provider, the Health Disparities Quality Improvement Organization Support Center will link you to an Every Diabetic Counts QIO in your state.
