Community Health Workers

Are you interested in making a positive impact on the public healthcare of your community? Are you interested in receiving free training on a diabetes self-management education curriculum? If you answered yes to these two questions, then we would love to work with you as a community health worker educating Medicare beneficiaries about managing their diabetes. As a Community Health Worker, the Health Disparities Quality Improvement Organization Support Center will help you receive diabetes education training and link you to an Every Diabetic Counts QIO in your state, so you can begin helping Medicare beneficiaries and their families.
