Throughout the United States and its territories, Medicare
Quality Improvement Organizations (QIOs) have been aggressively
engaged in evidence-based activities to reduce health care
disparities. These activities are to shape and alter pathways
that can ultimately lead to measurable improvements in health
care outcomes for Medicare beneficiaries. The QIO Program’s 9th
Statement of Work includes a Sub-National Task “Prevention:
Health Disparities” that seeks to provide diabetes
self-management education to Medicare beneficiaries.
Health
disparities are addressed throughout all three 9th SoW Themes:
Beneficiary Protection, Patient Safety and Prevention; in
addition to the Sub-national Tasks of Care Transitions and
Chronic Kidney Diseases. The Health Care Disparities QIO Support
Center (HDQIOSC) will focus on identifying individuals,
sensitizing providers, and adopting appropriate interventions
through evidence-based models and messaging.
